Using Fantasy Town style, I get this:

How do I get rid of all these weird entities. I did increase smoothing and levels trying to create a 'down'.

Is this just happening on export, or does it look like that inside CC3+ too?

Only on export. I notice that the flattened top of the 'hill' changes based on zoom level, as well. I guess that's normal?

If it changes based on zoom level, it sounds like you may have set the effect to be based on percent of view size instead of map units.

Looks to be some bands in the export, so unless you have already done so, increase your pixels per pass.
