Hide everything but your WALLS sheet, then use the BREAK command (lower left toolbar) to put a gap in both the room and the cave walls. Then show the other walls again.

Alternatively, load the DD3 Wall Feature symbols (click the DD3 button, then the Wall Feature catalog. Use the "Wall Cutter 10" symbol to cut a gap into your wall. You'll need to do it twice, since there are two walls overlapping at that point.

I'd perhaps use a more decorative font for the island names? As it is, it clashes a little with the style of the map. Why not use the same font as for the title, but keep the black color and smaller size?

Not sure why this should affect the menu file. Just re-run the latest update (Update 28) - that should fix the menu.

Campaign Cartographer does not open pdf files, only FCW files (CC3's own format). You don't happen the original FCW file lying around somewhere?

Yeah, it's just a note of the file type the pdf was printed from. Do you have Photoshop available? I'd try opening the map within that to see whether it can recover it.

Bu you can limit "Fill Basin with Lakes" by selecting an area first, right? That seems to work for me if I don't want to do it across the whole map.
