  It's possible those symbols are from an issue of the annual but I don't remember seeing them. That could just mean my memory's faulty, but I think those are custom symbols.

  There are some similar symbols in Annual 24. Squares, diamonds, circles, etc. for cities and 'major sites', along with smaller ones for lesser sites, etc.

  Yes. A couple of annuals have similar but fewer symbols. But I think MadCartographher made his own, or used others/augmented any from CC3. Again, just a guess.
