  It is certainly possible, but it takes a bit more work.
  First of all, there are no B&W house symbols in basic CD3, but when using the House tool in CD3, you can define your own houses and colors for these, so you can use this to make B&W houses for use with the house tool and random street tool in CD3.
  
  If you use mostly light-colored symbols in your map, you can also use a whole drawing RGB Matrix Process effect to make the entire thing grayscale, but if you use symbols with dark colors when making your map, turning it into grayscale this way will tend to make everything too dark and usually not very visually pleasing.

  Annual Vol 4 is also useable without City Designer. Are both together not better? Or rather just the Annual?

  You can use the Annual 4 issue without CD3, but then you won't be able to take advantage of the various city design tools like custom houses and random streets and more that comes with CD3, so the best option is having both IMHO.
