This is going to sound like a silly/obvious question, but is the image on the sheet that has the effect? Sometimes Layer and Sheet settings aren't where I think they are when I import — or even draw — stuff for my maps.
That aside, I would recommend against using effects while drawing the map. They can slow things down a LOT. An alternative might be to move whichever sheet you put the image on just below the land sheet. Then draw your land. Then move the image sheet above the land sheet and below the next one. It will obscure the stuff you just drew but you only need it for reference for the next level of drawings (contours maybe?). Once you draw/copy/trace that level of detail, move the sheet again. And so on. Just my two cents.
When I started the process I created a new layer named "trace". This layer was then moved so that all of the layers could be drawn on top of a transparency while still seeing what I was doing. I added the transparency effect to the trace sheet, but no amount of adjustment effected the imported image on it. The image was a png.

I have often noticed that some effects don't affect inserted bitmaps. In your case, a solution would be to use GIMP or PHOTOSHOP to edit your png picture, add a transparency (alpha) channel and save it as a copy. Then you insert the new picture.
