Have a strange thing happening here...shot of the symbol manager with the symbol file open for editing. Notice that Marsh and Swamp symbols are present.
I seem to remember reading somewhere that there is a bug in CC3 that doesn't show all symbols all the time. the way to get around this is to uncheck DISPLAY SYMBOLS at the top of Symbol Manager. While it is a little frustrating, once you know about it, it's no biggie.

The catalog is attached, of course you won't have the .PNG files. Some more information...
1) All symbols were created the same by taking a blank hex, cloning it and editing it. All should be on the Common Sheet. 2) The symbols always show up in the Symbol Manager with the symbol catalog is open for editing. 3) I have restarted the drawing countless times...if by restart you mean exit CC3 or load another file.
Hopefully Ralf will find a proper answer for you, but I did notice one thing. It matters which drawing you have open when trying to use the symbol catalog. I tried starting a new "CC3 Dungeon", and on this drawing, all the symbols show up when I load the catalog, but on a city template, I see the exact same issue with the same missing symbols as you report.
This is very likely a naming conflict. You have very generic names for your symbols like "Swamp" and "Desert", and some maps will have fill symbols with those names defined. As fill symbols are not displayed in the catalog window, it looks like the symbol are missing. To double-check, open the map where the problem appears, open the Symbol Manager and check "Show fill style symbols". There'll probably be symbols with the same names as your hex symbols.
There is a simple solution: rename your symbols to have a unique pre- or suffix. This is always a good thing to do, as it prevents naming conflicts, if you use different styles of symbols in one map.
I've attached a version of the catalog with renamed symbols.

Have you restarted the drawing? If you don't define these symbols on the Common sheet, they will only show up after a restart.
Do the symbols show up when you click "Show fill Style symbols"?
